1) Adaptive Student Information Form (SASEF Adapt Card).

There are two ways to obtain a "SASEF Adapt card" - fill it out and mail it to us or email it to us. Documentation may be required.

E-mail Option:
Click this link and we will email the Adaptive Student Information Form to you. You can open this file with Adobe Acrobat Reader. If you don't have Adobe Acrobat Reader, visit http://get.adobe.com/reader/ to download and install it for free. The Adaptive Student Information Form is an interactive form so that you can fill in the required information using your keyboard, save it to a file on your computer (using Adobe Acrobat Reader), then attach it on an email message to info@snowbasinadaptive.org.

Mail Option:

Then send the completed form to SASEF, P. O. Box 1193, Eden, UT 84310.
